A case of ischemic stroke with negative series of vascular workup is reported. A 72-year-old male with a history of hypertension was admitted to our hospital for evaluation of acute right hemiplegia. He had taken a promethazine after several febrile episodes in the past. He had no history of smoking or alcohol drinking. Neurological examination revealed that the right side of the body was flaccid with complete aphasia. He was unable to move his right extremities and speech was difficult because of dysarthria. A large, mixed venous infarction was observed on brain computed tomography (CT). Angiography showed complete occlusion of the right internal carotid artery and the middle cerebral artery (MCA) bifurcation. Complete revascularization was performed, however, an early reocclusion of the right ICA was observed. He did not take any antiplatelet agents, and anti-coagulants were not given.
